Using States instead of Countries- how to do it?

Our dolphin community is under customization to be launched soon. 99% of members are from one county- (that's county, not countRy).
Dolphin is based on country locations unfourtunately. This would be totally useless to us and we'd like to modify it to be changed to a city basis, rather than countries.
Any suggestoins to how to do this and if there is more than one place to do it (ie, For events and profiles, etc.) ?
