Similar to Schools mod

Hi,

I'm looking for a mod similar to this:

I have sent modzzz a few emails but have not heard back so I'm opening it up to the community.  The school mod currently requires a school name in order for the search to work.  I need it so people can search a city/state and not require school name.

I would also like a claim listing link and functionality to use paypal for payments
