Join Form fixes and confirmation upgrades

I am having some trouble with my join form listed below

-email confirmation needs to work
-Join Form does not work at all
-Join button on Join form needs to just read "Join Now"
-email, phone number, and zip code verification using reg expressions
-phone number format needs to be limited to 999-999-9999
-max username length to be 12
