I could not find the 'Membership' block and I am not sure how to create one!

Stuart this is 6.1.1 when I say 6.1 I mean .1.1 cause I assume all who used .1.0 upgraded...not many stayed with orig.....its there....check upgrade sql

You can check the DB open phpmyadmin and look in ProfileFields and see if they are there...
