Username/Email already used by another member

when user join my site, Username field and email field error like this Username/Email already used by another member.

and not retrive to Avator page please help.

Using ver:7.1

It means that another member is already using that username or email, and as such, they cannot join with it. Unless of course there isn't a previous member, then that means we have a problem on our hands.
