Creating User Form and embed into new page

Has anybody ever created a form in html or 3rd party form creator and embedded into blank page? Something like Mail Chimp or Google Forms? 

The problem would be:

1. Getting the form to communicate with the database.

2. Getting the database to deal with that data and output it in a meaningful way.

Both of the above would require a bit of code.

Do you have something specific in mind?

I'm pretty sure you could stick a Bravenet form in there.
