custom memberships level

is there  a way when you create a custom membership level instead of being paid, that a member can only get this level if admin approves it/changes to it manually

Yes.  Create the membership but don't make it purchasable.  Then you can log into the member's account and assign the membership manually and set the expiration for the membership.  The easiest way is to go to admin, members, and click on the link to the right of the avatar/icon for the member.
